Overview
Staff in the Office of Population Health partner closely with community organizations and other healthcare partners to develop programming to address our most pressing community needs that are impacting the health of our community. This work is guided by our Community Health Needs Assessment.
We work in partnership with our colleagues in diversity, equity and inclusion and community relations on initiatives that promote health equity.
Reports
Community Health Needs Assessment (CHNA) looks at the health of a community by using data and collecting community input. CHNAs examine community health from a big-picture view and consider risk factors, quality of life, mortality, morbidity, access to health care and more. They assist in establishing priorities for community health and developing and implementing community health programming.
UW Health CHNA priorities are maternal and child health, chronic conditions, substance use disorders, mental and behavioral health.
