Breah Klemp, PA, is a UW Health family medicine physician assistant. She provides home-based primary care as part of a medical team that cares for individuals in their homes.

Breah treats people with numerous chronic (ongoing) conditions, including heart failure, diabetes, high blood pressure, atrial fibrillation, dementia, insomnia and pain. She also treats any sudden illnesses or issues they may have, such as bronchitis, ear infections, headaches, flu and musculoskeletal (muscle and bone) injuries.

To help keep her patients safe at home, Breah also educates them about fall prevention.

Her special interest is helping patients navigate the health care system so they can get the care they need.

During her free time, Breah enjoys spending time outdoors with her family, sports and participating in church activities.

I believe in treating each patient as a person first — addressing their concerns, answering questions and helping them make informed health care decisions.
