Our home health agency provides skilled care and in-home assistance from nurses, therapists and others. We work together to help you achieve your health goals.
Who are the Home Health clinicians?
Skilled nurses assess patient's needs after a hospital or nursing home stay, address changing medical conditions, provide skilled nursing treatments such as IV therapy, wound care, and medication management, educate patients on health management and treatment and coordinate patient care with other services.
Physical therapists assess and treat patients who have problems with mobility, falls, home safety issues and/or training needs with new equipment.
Occupational therapists assess and treat patients who need cognitive testing or help with daily living skills, such as dressing, bathing and homemaking.
Speech and language pathologists evaluate and treat speech and swallowing issues.
Social workers evaluate and counsel on social, emotional and economic needs related to medical problems.
Home health aides assist patients with bathing, hygiene and personal care during a skilled home health episode.
Intake nurses can be reached seven days a week at (779) 696-7550 to make a referral. The intake nurses interpret the home health coverage criteria and verify coverage. Most private insurance plans, Medicare and Medical Assistance provide coverage for skilled home health agency visits for eligible patients.

Business hours are 8:30 a.m.-5 p.m., Monday through Friday
On-call clinicians are available 24 hours a day, seven days a week
Aide services are available 8:30 a.m.- 5 p.m., seven days a week
Therapy services are available 8:30 a.m.-5 p.m., Monday through Friday
Social work services are available 8:30 a.m.-5 p.m., Monday through Friday
We offer a full line of infusion medications that can be given in the home setting. Our staff included a high-tech team of pharmacists and nurses who specialize in home infusion.
If you require tube feedings, we can help. Our registered dietitians will assess your needs and make recommendations. We provide supplies and education for you and your caregivers.
If you need additional training, we will coordinate with a local home health agency to teach you how to use the tube feeding at home.

Medicare, Medicaid and most private insurance plans offer coverage for many of our services. In some cases, they cover 100% of costs. In other cases, they cover part of the cost. We also help you understand your insurance coverage and what out-of-pocket charges to expect.
